Home Health Services

Skilled Nurse:

Develops and carries out the plan of care for each patient. Services include skilled care and assessment of patient needs, diagnosis-specific education for patients and caregivers, and coordination of care with the patient’s primary health care doctor.

Physical Therapist:

Provides assessment and training in transfers, mobility, gait, pain, therapeutic exercise, strength and instruction on equipment and home safety precautions.

Occupational Therapist:

Conducts assessments and trains patients in daily living activities of feeding bathing, and dressing.

Speech Therapist:

Provides assessment and training for patients who have speech, language, voice, swallowing and cognitive disorders.

Medical Social Worker:

Conducts assessments and provides case management assistance to patients and caregivers. The MSW also makes referrals to appropriate community resources and provides short term planning needs.

Certified Home Health Aide:

The Aide provides in-home personal care duties such as bathing, assistance with meals and activities of daily living.
